Sutter Care at Home is the home health care and hospice affiliaThe role of the LVN Team Care Coordinator (TCC) is to provide clinical support related to the processing of home health orders and Durable Medical Equipment. The goals of the LVN Team Care Coordinator are to support field clinicians' work, team communication, and assist with the processing of location specific EHR orders under the supervision of the Director of Patient Care Services. The TCC will support the Home Health team through coordination and communication with patients, physicians, and other clinicians as appropriate. The key objectives are to eliminate unnecessary and unwanted hospital readmission, to provide high patient and provider satisfaction, and to improve the quality outcomes for those patients on service with Home Health.
Primary clinical responsibilities include receiving/processing Epic order requests generated from field staff and responding physicians, DME order processing and tracking and telephonic support for the Clinical Supervisor team. The LVN Team Care Coordinator enters the HCHB Epic Order Request Coordination note into Epic and then transcribes medical provider's response into HCHB in the form of an order or communication note. As the LVN Team Care Coordinator, this person frequently interacts with referral sources and physician groups, other members of the Home Health team, and all others as necessary to facilitate coordination of care. This role utilizes and promotes use of excellent customer services skills when working with referral sources, patients and branch staff.

Must have a current, valid California LVN license.
Minimum of one year clinical experience required, with a minimum of two years preferred.
Experience in coordinating/processing home care orders and/or at least one year's experience in a home health agency or community health setting strongly preferred.
Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the supervisor. Must have knowledge of current Home Health Medicare, Medi-Cal, State and Federal regulations.
Special Skills /Equipment:
Must be able to demonstrate problem-solving abilities and have excellent telephone, interpersonal and written communication skills. Strong commitment to teamwork and customer service, both internally and externally, is required. Must have strong data entry and general computer skills.
