The Neogen Care Home Health RN leads and monitors patient overall health and treatment, under the direction of a primary care physician, for the entire period of at home treatment; with the goal of returning the patient to functional independence in the shortest time possible per safety and treatment guidelines. The RN manages cases by develop individualized plans of care for their patients, and manages all care provided by clinicians and therapists. The Home Health RN provides specialized services including customized care, patient education, ostomy care management, wound therapy, disease management, and post-surgical care.
We provide our RNs outstanding support from our own internal operational support team to handle tasks such as: pre-populating medication reconciliation, tracking DME orders, obtaining MD signatures, coding, lab follow-ups, and many others. This support system enables our RNs to function remotely and independently with the flexibility to make schedule and plan patient visits around other commitments if needed.
A sample of duties and responsibilities for this position include:Providing admission, case management, and follow-up skilled nursing visits for home health patients.Administering on-going care and case management for each patient, providing necessary follow-up.Conferring with physicians in developing initial plans of treatment based on physician's orders and initial patient assessment.Providing hands-on care, management, and evaluation of the care plan and teaching of the patient in accordance with physician orders, under a Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulations.Coordinating appropriate care, encompassing various healthcare personnel (i.e. Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).Reporting patient care/condition/progress to physician and Clinical Manager on a regular basis.Implementing patient care plans in conjunction with patients and family members to assist them in achieving optimal resolution of needs/problems.Coordinating/overseeing/supervising the work of Home Health Aides and Licensed Vocational Nurses and provide written personal care instructions/care plans that reflect current plan of care.Monitoring and ensuring the appropriate completion of documentation by Home Health Aides and Licensed Vocational Nurses.Discharging patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.Preparing appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.Preparing visit/shift reports, updating/summarizing patient records, and conferring with other health care disciplines in providing optimum patient care.Other duties as assigned by the Clinical Director via a Clinical Manager.
REQUIRED SKILLS AND EXPERIENCE:A minimum of 1 year of recent acute care experience in an institutional setting.A minimum of 1 year of recent Home Health Agency experience with relevant experience in wound care, patient teaching, medication management, lab draws, and IV infusion.Diploma, Associate, or Undergraduate degree in Nursing.Current and unrestricted Registered Nurse licensureCurrent CPR/BLS certification.Must be a licensed driver with vehicle, in good working order, that is currently registered and insured in accordance with state and Neogen Care requirements.Strong skills in observation, verbal and written communication, problem-solving, basic math, Electronic Medical Record Software (Wellsky/Kinnser a plus), and basic computer skills to manage email, texts, employment record self-service, etc.Visual acuity and hearing to perform required nursing skills.
