This licensed is responsible optimal delivery of pharmaceutical care for patients eligible for the oral chemotherapy drug treatment. This role provides high quality care and requires a high degree of interactions with City of Hope physicians and patients. The Pharmacist will assisting in performance improvement initiatives, participates in the supervision of human and material resources, as well as program planning and evaluation. It is expected that the individual who holds this position is able to actively participate with peers, subordinates, supervisors, and the care team in constructive problem solving to facilitate the best outcome for the patient.* Essential Functions:* Creation and management of Foundation-wide drug formulary. Develop and manage therapeutic interchange to achieve optimum cost-benefit drug utilization.* Build and maintain chemo regimens in Intellidose. Manage regimens to be evidence-based per NCCN recommendations to ensure compliance with payers in order to achieve reimbursement.* Manage contracts to ensure optimal rebates.* Maintain drug inventory nucleus library with correct billing codes to interface with practice management system pulse.* Maintain nursing competencies with IV drug admixing* Oversees and is actively involved in the Medication Therapy Management billing process, medication reconciliation, and the REMS program as mandated by the FDA.* Provides education to patients on the proper use of any prescribed medication.* Trains and educates staff on the safe handling of hazardous drugs and materials, ensuring proper steps are taken by staff to reduce likelihood of injury or illness.* Supports and promotes the efforts of all care providers to deliver quality care.* Promotes the delivery of quality care consistent with the scope of practice of the discipline.* Ensures and promotes adherence to professional practice standards through education, training, coaching and monitoring.* Participates with the Director and others in the implementation and evaluation of new services/programs.* Supervise personnel and personally delivers pharmaceutical care and service/ information/ medication/ products by serving as a role model and through effective use of change-agent strategies.* Adhere to all professional and regulatory standards.* Promotes collaboration and teamwork among all members of the healthcare team.* Effectively communicates/educates all customers to meet organizational goals and customer service expectations.* Maintains current knowledge, skills and competencies through involvement in professional continuing education activities.* Manages/supervises the efficient and effective utilization of resources and staff* Fosters involvement in performance improvement activities to promote continuous improvement in care delivery.* Demonstrates assimilation of core organizational values within own performance.Follows established City of Hope and department policies, procedures, objectives, performance improvement, attendance, safety, environmental, and infection control guidelines, including adherence to the workplace Code of Conduct and Compliance Plan. Practices a high level of integrity and honesty in maintaining confidentiality.Performs other related duties as assigned or requested.Position Qualifications:Minimum Education/Experience:* Graduate of an ACPE-accredited School of Pharmacy with ACPE accreditation pending with a BS Pharmacy degree* Experience may substitute for minimum education requirementsReq.
